How Does It Work
Atovaquone is a selective inhibitor of parasite mitochondrial electron transport. Proguanil hydrochloride primarily exerts its effect by means of the metabolite cycloguanil, a dihydrofolate reductase inhibitor. Inhibition of dihydrofolate reductase in the malaria parasite disrupts deoxythymidylate synthesis. Together, atovaquone/cycloguanil affect the erythrocytic and exoerythrocytic stages of development.

Before starting Atovaquone and Proguanil, Malarone makes sure your physician is aware of any allergies or medications you currently take. Elevated liver enzymes have been reported with prophylactic use of Atovaquone and Proguanil, Malarone.
Usual Dosing
Oral (Prophylaxis): Take 1 tablet (atovaquone 250 mg/proguanil 100 mg per tablet) once daily; start 1 to 2 days prior to entering a malaria-endemic area, continue throughout the stay, and for 7 days after returning.
Oral (Treatment): Take 4 tablets (atovaquone 250 mg/proguanil 100 mg per tablet) once daily for 3 days. If used for severe malaria (after completion of IV therapy), use a full 3-day schedule (CDC 2020).

How to Take: Take with food at the same time each day. If vomiting occurs within 1 hour of administration, repeat the dose.
Side Effects
Diarrhea, anorexia, abdominal pain, nausea, vomiting, elevated liver enzymes, pruritis, weakness.

What do I do if it really upsets my stomach?
Try taking it after meals. It is not required however it may lower risk of upset stomach, nausea, or diarrhea.
Do I have to take this medication every day?
Do not stop taking this medication, it is very important to take as prescribed for the full duration if used prophylactically to avoid infection. If halted early or if dosages are missed, malaria risk increases. If used for treatment, take as prescribed to prevent the parasites from forming a resistance to the medications.
