How Does It Work
Bromocriptine, Cycloset/Parlodel is an ergot alkaloid derivative and a dopamine agonist which binds to post-synaptic receptors to inhibit pituitary prolactin secretion and enhance coordinated motor control. Prolactin is best known for stimulating lactorrhea but also has been found to be linked to sexual libido and penile erection. Studies have shown that the neuroendocrine response to sexual activity is characterized by a pronounced orgasm-dependent increase in prolactin levels. Men who have higher prolactin levels are likely to experience lower sexual drive and erectile dysfunction Bromocriptine, Cycloset/Parlodel has been shown to have a positive impact in the treatment of sexual disorders as well as help improve overall sexual performance by reducing the duration of the refractory period, improve sexual libido and arousal and improving overall penile erectile function. In addition, Bromocriptine, Cycloset/Parlodel is believed to reset the hypothalamic circadian rhythm which can reverse insulin resistance and decrease glucose production in obese patients along with maintaining normal leptin stimulation during caloric restriction which will normalize the metabolic rate.
Indications For Use
Bromocriptine, Cycloset/Parlodel is officially indicated for use in hyperprolactinemia (excluding Cycloset), Parkinson’s disease (excluding Cycloset), and Diabetes mellitus, type 2 (Cycloset only). It is also prescribed off-label for use in neuroleptic malignant syndrome, anorgasmia, and to stimulate metabolic rate during caloric restriction.
Precautions
Can cause cardiac valvular fibrosis
Can cause CNS depression and should be used with caution in patients that require mental alertness in performing tasks
Can reduce impulse control and has been associated with compulsive behaviours
Can cause pleural/retroperitoneal fibrosis
Usual Dosing
Hyperprolactinemia (excluding Cycloset)
Oral: Start with an initial dose of 1.25mg to 2.5mg daily; dose may be increased by 2.5mg daily as tolerated every 2 to 7 days until optimal response is reached (therapeutic range: 2.5mg to 15mg daily)
Parkinson disease (excluding Cycloset)
Oral: Take 0.8mg once daily in the morning; dose can be increased by 0.8mg weekly as tolerated (therapeutic range: 1.6mg to 4.8mg daily)
Diabetes mellitus, type 2 (Cycloset only)
Oral: Take 1.25mg twice daily, dose can be increased by 2.5mg daily in 2 to 4-week intervals as needed to a maximum of 100mg daily.
Anorgasmia (Off-Label)
Oral: Take 1.25mg to 2.5mg twice daily.
Metabolic Rate (Off-Label)
Oral: Take 1.25mg to 2.5mg twice daily.
Pharmacist Tips On Using
How Do You Take Bromocriptine, Cycloset/Parlodel: Cycloset should be taken within 2 hours of waking up in the morning. Bromocriptine, Cycloset/Parlodel should be taken with food to decrease any GI upset. Take the medication exactly as prescribed by your doctor at the same time each day. If you miss a dose, make sure to take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and continue with your regular schedule.
Do Not Use Bromocriptine, Cycloset/Parlodel: If you have a known hypersensitivity to Bromocriptine, Cycloset/Parlodel or any component of the formulation; do not use Cycloset if you have syncopal migraines and/or breastfeeding. Do not use Parlodel if you have uncontrolled hypertension and/or pregnant.
Side Effects
The frequency of side effects can vary based on the dose and/or indication used:

Gastrointestinal: Constipation (11% to 14%), nausea (18% to 33%)
Nervous System: Dizziness (≤13%), headache (≤13%)
Neuromuscular and Skeletal: Asthenia (13%)
Respiratory: Rhinitis (14%)
